Add Passport Pages

It is easy to add pages to your U.S. passport through Passport & Visa Exchange's rush service but if you need super rush service then you must click here for special instructions. Otherwise, here is what you'll need to do:

Fill out the State Department application form DS-4085. It MUST be completed ONLINE. Please click now for the DS-4085 and follow all directions provided.

Send this application to Passport & Visa Exchange together with the following:

1. Your valid U.S. passport;
2. A check made out to the US Department of State for $60;
3. One authorization form (click to print);
4. One copy of our Traveler Information and Payment Form (click to download).
5. If appropriate, one copy of your ticket or itinerary showing proof of travel date and your name (if no travel upcoming just ignore this step);
6. Passport & Visa Exchange Special Discounted Fee
      (Visa/Mastercard, money order/cashiers check/business check)
      (our fee is in addition to any required State Department fees)
     $  145.00 -- express service - 4 days to 3 weeks
     $    95.00 -- standard service - 3 weeks or more
   (for 1 to 3 day service click for our Super Rush option)
   Add $28.00 shipping to all orders and an additional $18 surcharge for Saturday delivery.
   If, for any reason, the Passport Office suspends your passport, a reprocessing fee will be charged.
