Change Name on Passport - Super Rush Service

It is easy to change the name on your valid U.S. passport through our super rush service. However if your passport was issued MORE THAN one year ago you'll have to go through the renewal process as well.

If you need faster service then you must use same day service; click here for special instructions.

If your passport is less than a year old please fill out and print the re-application form DS-5504. Note that if your passport is more than 1 year old (and less than 15) then you must go through the passport renewal process. You must apply for a new passport if your passport is more than 15 years old. Please Click Now for the DS-5504 and follow all directions provided.

Send the SIGNED DS-5504 applications to Passport & Visa Exchange together with the following:

Your valid U.S. passport;

2 professionally-taken recent passport photos - do not use kiosk or your own photos;

Include a check made out to the US Department of State for $60.

Proof of name change; either a Court-Certified Name Change Decree OR a Marriage Certificate (not a Marriage License) OR a certified copy of a Divorce Decree.

Copy of the front and back of a current state photo ID (i.e. your drivers license).

One authorization form (click to print);

One copy of our Traveler Information and Payment Form (click to download).

One copy of your ticket or itinerary showing proof of travel date and your name;

Add $29 shipping to all orders and an additional $20 surcharge for Saturday delivery.
