Change Name on Passport
It is easy to change the name on your valid U.S. passport through
Passport & Visa Exchange's rush service. However if your passport was
issued MORE THAN one year ago you'll have
to go through the renewal process as well. If you need super

here for special instructions. Otherwise, here is what you'll need
to do:
  If
your passport is less than a year old please fill out and print the re-application form DS-5504. Click here to open form, which must be filled out ONLINE. AFTER filling out the form online, print, sign and date it. Note that if your passport is more than 1 year old (and less than 15) then you must
go through the passport renewal process.
If your passport is more than 15 years old then you must apply for a
new passport.
  Send
the SIGNED
DS-5504 applications to Passport & Visa Exchange together with
the following:
- Your valid U.S. passport;
- 2 professionally-taken recent passport photos - do not use kiosk
or your own photos;
- Include a check made
out to the U.S. Passport Service for $60.
- Proof of name change; either a Court-Certified Name Change Decree
OR a Marriage Certificate (not a Marriage License) OR a certified
copy of a Divorce Decree.
- One authorization form (click
to print);
- One copy of our Traveler Information and Payment Form (click to download).
- If appropriate, one copy of your ticket or itinerary showing proof
of travel date and your name (if no travel upcoming just ignore this
step);
- Passport & Visa Exchange Special Discounted Fee
(Visa/Mastercard, money order/cashiers check/business check)
$ 145.00 -- express service - 4 days to 3 weeks
$ 95.00 -- standard service - 3 weeks or more
(for 1 to 3 day service click for

Add $26.00 shipping to all orders and an additional $15 surcharge for
Saturday delivery.
If, for any reason, the Passport Office suspends your passport, a reprocessing
fee will be charged.
